IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
Shooting Star Casino is providing you with information regarding your 2025 health coverage. The IRS Form 1095-C, “Employer-Provided Health Insurance Offer and Coverage,” is available for all employees/past employees who were eligible for health coverage during 2025. We are required to provide information regarding your health insurance coverage (Form 1095-C) to the IRS. While we are not required to mail a hard copy to your home, a copy is available to you upon request. To receive your 2025 Form 1095-C, please contact the HR department at gbranchaud@starcasino.com or call 218-936-2537. If you are an active employee, you may also access your form electronically via UKG through October 15, 2026.
